Recently, you may have received a letter from Public Health – these notification letters were sent to parents/guardians of students, or students over the age of 16, with incomplete vaccination records. The Immunization of School Pupils Act (ISPA) requires that all students in Ontario receive certain vaccines or provide a valid exemption form to attend school.

If you received a letter from Public Health, you may need to:

• Book an appointment with your doctor to receive required vaccinations
• Submit vaccination records to Public Health online at www.regionofwaterloo.ca/vaccines
