Dear parents, guardians and students,
If your secondary student received a letter from Public Health, choose from the following options to update their vaccination record:
- Complete this form to report any vaccines that are listed on the suspension order that the student has already received
- Book an appointment with your health care provider and complete this form to report any vaccines they have received.
- Note: Public Health is processing a high volume of record submissions. You will receive a confirmation email as soon as your vaccination records have been processed. Please do not call or resubmit your records.
If you do not have a health care provider:
- Public Health vaccine appointments for secondary students who have received a letter from Public Health can be booked online here.
- A clinic will take place at your school on February 26, 2026, use this link to register.
